The Federal University of Health Sciences, Azare (FUHSA) has announced the requirements, procedures and applicable fees for students seeking intra-university change of course or inter-university transfer.
The information was contained in a circular issued by the Office of the Registrar and dated 10th August 2026, following the approval of the modalities by the University Senate at its 42nd meeting held on the same day.
According to the university, interested applicants are required to visit the application portal at https://applicant.safapply.com/fuhsa, complete the appropriate application form and submit it alongside relevant documents. Applicants must also possess at least five O’Level credits, including Mathematics and English.
The transfer is subject to the availability of vacancies. Applicants are also required to attach their academic transcripts, obtain the necessary departmental, faculty and university clearance/release, and secure acceptance from the receiving department and relevant authorities.
For academic requirements, candidates seeking transfer into MBBS and BDS must have a minimum CGPA of 4.01, while candidates applying for Nursing Sciences and other programmes must have a minimum CGPA of 3.50.
Approved Non-Refundable Fees
- Change of course form: ₦50,000
- Acceptance fee for change of course within FUHSA: ₦250,000
- Transfer from public universities in Nigeria: ₦500,000
- Transfer from private universities in Nigeria: ₦2,500,000
- Transfer from foreign universities: ₦3,000,000
FUHSA further stated that applicants may be required to take prerequisite courses from the receiving department where applicable. Candidates must also regularise their transfer with JAMB and complete an indemnity form.
The university added that a candidate cannot be transferred more than once.
